The attestation a notary asks for, filled in one go.

An owner sells, their notary writes to the association, and the board has fifteen days to produce a document whose exact contents nobody knows. Its contents are set by decree 991-2025, which lists eight items. We grouped them into five headings, in the order of the regulation.

The request

Who attests, and on what date. The article 1068.1 deadline runs fifteen days from the co-owner's request.

1. Identification of the private portion

The unit being sold, as the notary will look it up in the land register.

This unit's share of common expenses. The notary checks it.

2. Financial information

The state of the reserve fund, available cash, and what the seller still owes the association.

Write 0 if the seller is up to date. An empty field reads as an omission.
A budget is a document, not a figure: the regulation asks for the document itself. Attach it to the PDF before handing it over.
Surplus or deficit of recent financial years
Contributions to common expenses over the last three years, all co-owners These are the charges for the WHOLE condominium, not for the unit being sold. The unit's arrears go in the previous field.

3. Insurance information

The article 1073 coverage, the self-insurance fund and the highest deductible.

4. Legal information

Ongoing litigation and amendments to the declaration of co-ownership over the last three years.

5. Information about the building

Inspections and claims over the last five years, major repairs, and work planned over ten years.

Write "None" rather than leaving it empty: an empty heading does not say whether you have nothing to declare or did not look.

Read this before handing it over

This document is unsigned, and it commits only to what you wrote in it. It must be dated and signed by the association's authorised representative, with their name and role, before it goes to the notary. DO NOT FORGET TO ATTACH THE PROVISIONAL BUDGET for the current year: the regulation requires it, and this PDF cannot contain it. Several headings require your own records: the fund balance on the exact date, contributions to common expenses over the last three financial years, the logged maintenance work. No form can guess those for you.
